Wandsworth Professional Referral Form

 Referrals are accepted from people aged 18 years and over who are:

  • Wandsworth Residents, or
  • Non-Wandsworth residents but who are registered with a Wandsworth GP

 

We can help if you are experiencing anxiety or stress, depression, bereavement or loss, sleep difficulties, anger issues, obsessive compulsive disorder, phobias, eating disorders and many other everyday problems.

 

Professional Referral Form

Referrals from GP or any Health Care Professional who work in a GP practice and can refer via Electronic Referrals Service (e-RS) Please note: *From May 2022 the e-RS route will be the only way for GP Practices to send in referrals. This means that all first outpatient referrals must come into our Trust via the Electronic Referral Service (e-RS).
